QUALIFICATIONS:

• Gender: Male & Female
• Bachelor of Science in Physical Therapy w/ Professional license
• Age: 21 – 38 years old and below
• with at least 1 yr. exp (not more that 6 months gap in work experience)

• Diploma
• Transcript of Records
• Related Learning Experience
• PRC Board Certificate
• PRC Board Rating
• PRC license
• Passport valid for more than 2 years (old & new)
• Pictures (passport-sized, with white background, newly taken)
• PSA Marriage Certificate (for female only, if applicable)
